State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.

Information about health care providers is on the agency’s website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health home page (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Pierce County

In November 2022 the secretary of health suspended the independent clinical social worker associate license of Jeanette Alva Carter (CG60266129, SC60868758). Carter, formerly a psychiatric social worker at a Washington corrections facility, engaged in inappropriate sexual communications with a patient. She did not respond to charges.

Spokane County

In October 2022 the secretary of health suspended the registered nursing assistant license of Domanic Russell Shaver (NA61068384). Shaver stole items from an in-home client’s safe and pawned them. He was charged with theft in Spokane County Superior Court in January 2022. He also sent a picture of his genitals to another caregiver working in the home.

Thurston County

In October 2022 the secretary of health revoked the home care aide license of Stephanie Kaye Gerrer (HM61030487). Gerrer was convicted of felony assault of a child in Thurston County Superior Court.

Whatcom County

In October 2022 the secretary of health revoked with no right to reapply the certified nursing assistant license of Joshua John McDonald (NC61000690). McDonald was booked into jail in April for attempted murder and malicious mischief after stabbing his patient in the neck and cutting her throat with a kitchen knife.

Yakima County

In October 2022 the Pharmacy Commission and Susan LeeAnn Beebe (VB60818015) agreed to an order suspending her pharmacy assistant license for at least three years. The Department of Social and Health Services’ Adult Protective Services determined that Beebe had financially exploited a vulnerable adult and prohibited her from caring for vulnerable adults.

In October 2022 the secretary of health revoked the home care aide license of Uriel Badillo Guerrero (HM60543218). Guerrero was convicted of felony assault in Yakima County Superior Court.

In October 2022 the secretary of health summarily suspended the registered nursing assistant credential of Samuel Gonzalez (NA61163483). The Department of Social and Health Services’ Adult Protective Services determined Gonzalez abused a vulnerable adult.

In October 2022 the secretary of health suspended the substance use disorder professional trainee license of Roxanne Sanchez (CO61036095). Sanchez did not respond to an investigator seeking an explanation in response to allegations that she attempted to adulterate a workplace drug test, attempted to obtain a substitute urine sample, entered a car loan transaction with a patient, and borrowed money from other patients and co-workers.

In October 2022 the Nursing Assistant Program suspended the certified nursing assistant license of Jennifer Suzanne Taylor (NC10051366). Taylor failed to comply with a 2021 Agreed Order that required her to provide her employer with a copy of the order and submit quarterly performance reports.
