State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.

Information about health care providers is on the agency’s website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health home page (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Chelan County

In July 2022 the Agency Affiliated Counselor Program suspended the agency affiliated counselor license of Suyedid Lozano Vargas (CG61079169). Vargas failed to comply with a 2021 agreed order that required substance use monitoring program enrollment.

Grant County

In July 2022 the Board of Osteopathic Medicine and Surgery and Allen D. Quinn (OP00001193) agreed to an order in which Quinn surrenders his license to practice as an osteopathic physician and surgeon, including any temporary, emergency, or volunteer practice. He has no right to reapply or reactivate his credential. Quinn violated the terms of a 2013 agreed order by treating and prescribing controlled substances to non-hospice patients.

Mason County

In July 2022 the Nursing Commission suspended the licensed practical nurse credential of Kristan Kelly Ramirez (LP60259260). Ramirez failed to respond to charges that she was out of compliance with a required substance use monitoring program.

Snohomish County

In July 2022 the secretary of health suspended for at least six years the massage therapist license of Brandon Lee Groesbeck (MA60313574) after he was convicted of indecent liberties, a felony sex offense, in Snohomish County Superior Court. Groesbeck did not respond to the charges.

In July 2022 the Nursing Assistant Program suspended registered nursing assistant Sarah Marie Nichols (NA61136314) for failing to comply with a 2021 agreed order that she enroll in a substance use monitoring program.

In July 2022 the Board of Osteopathic Medicine and Surgery and Dennis Steven Mann agreed to a modification of a previously agreed order he surrenders his osteopathic license (OP00000949). Mann agreed to never resume practice, including temporary, emergency, or volunteer practice and to not seek reinstatement. Mann and the Board agreed to a 2021 stipulated order after he failed to document in examinations whether opioid treatment was necessary for several patients. Mann requested modification of the 2021 agreed order to surrender his license.

Spokane County

In June 2022 the secretary of health revoked the home care aide license of Charlene Perez (HM60708673). The Department of Social and Health Service’s Adult Protective Services determined Perez financially exploited a vulnerable adult.

Out of State

Michigan: In July 2022 the Nursing Commission summarily suspended, pending further disciplinary action, the registered nurse license of Laurie Ann Pickard (RN60989965). The Texas Board of Nursing had entered an agreed order with Pickard that placed Pickard’s nursing credential into inactive status after Pickard violated a Texas Board of Nursing order. Based on the disciplinary action in Texas, the Wisconsin Board of Nursing suspended Pickard’s license, and the Florida Board of Nursing accepted Picard’s voluntary relinquishment of her license.
